Overview

SCDD is seeking grantees to train and support implementation of Supported Decision Making (SDM) among families/individuals and professionals within these five (5) focus areas: Educational Services, Financial Services, Intellectual/Developmental Disabilities (I/DD) Services, Legal/Judicial Services, and Medical/Healthcare Services. The Council has a total of $3,000,000 available to fund a maximum of five (5) SDM-TAP grant projects in two (2) areas of implementation: professional and family/individual. The Council will consider all proposal submissions and choose one (1) or more, but no more than five (5), with a minimum award of $150,000 and a maximum award of $800,000 per application that will not exceed the full $3,000,000 budgeted amount, subject to funding availability. The intended...
